I was wondering if anyone else was having troubke accessing the Tuberculosis Risk Assessment questionnaire. They gave me directions on where to find it in the Wellness Portal and I genuinely cannot find it anywhere. It’s not under Forms or anything, and I’ve clicked every possible hyperlink on the website at this point. Please help
