Why is the M.E.M. so janky to control?

This is my lander, I consider myself experienced regarding to Mun landings since this is my 18th landing. I find the MEM hard to control since it moves from side to side and can't stay stable with stability assist enabled. I have reaction wheels, a SAS probe and I still find it difficult to maneuver.

How do I fix this?

I am sure that the lander - orbiter part is too heavy, what can I do about it though?

If my lander has 1500v and my orbiter has 3200v, is that enough to travel from Kerbin orbit to Duna orbit and return?
