Good Sunday afternoon everyone. Hope the weather is as great where you are as it is here in my little corner of the PNW. I need some advice from you T1 experts.
I was dx almost a year ago after immunotherapy for cancer turned my immune system into an enemy & it destroyed my pancreas & thyroid overnight. Went unconscious from DKA & woke up 2 days later in ICU with no clue what happened to me. No history of diabetes until this. No family history either.
It's been a rough ride. I am on Lantus 35 mg daily & premeal boluses of Novolog 1u per 10carbs in am & 1u per 15 carbs other times. I wake up most days around 150 & my sugar goes screeching upwards about 100 points more as soon as I stand up. Even doing some Novolog while still lying down doesn't help. It also seems as if the Novolog I take during the day doesn't work at all until 5 or 6 pm. It's to the point that I don't eat during the day because my sugar is already high & Novolog doesn't control it. Since I know the Novolog seems to kick in around early evening I'll eat something about an hour before so I don't crash but don't take novolog to cover the carbs.
I think my Lantus doesn't last 24 hours in my body. I think it covers maybe 18 hours. My MD is opposed to splitting the dose to 2x daily instead of daily. I usually take it around noon or so which is about 6 hours before I see any kind of response to the insulin I've taken during the day. He tells me that I'm doing well as my A1C is around 7 & it was over 12 when I wound up in ICU. I also have a scale of 2 units of Novolog for each 50 points my glucose is above 150. This starts at 150-200 & goes up 2 units for each 50 points more.
I've tried exercising as much as I can, drink lots of water & nothing seems to help. Health issues restrict how much I can do physically, which isn't a lot compared to the pre cancer me. My immune system is now attacking my muscles. Obviously I couldn't do the 80mg daily prednisone first prescribed for this due to type 1. I took the first dose & it took me days to get below 300. I'm getting this stuff called IVIG one week every month. I don't know it this affects the type 1 diabetes or not.
Anyone have any advice that I can try to see if I can get more in range? I aim for under 150 but with my sugar shooting upward first thing in the am & staying up all day I rarely hit that mark. Thanks eyeryone.
