Later on during another hallucination, it's Karen wearing the same sweater. Anyone know what the significance was? I couldn't tell what else was on the sweatshirt since I couldn't screenshot it.

I mean I kinda figure it's that Frank subconsciously sees Karen in some kind of wifey role in his mind. It was also intimated during her psych eval with Heather on Born Again that something is or was happening between them that Matt doesn't know about. Heather straight up asks if "Matt knows" after Karen's highly charged reaction to Heather mentioning Frank. So showing Maria and then Karen wearing the exact same sweatshirt seems like there is something else going on. !<
