Clomid yes or no? NOA pre mTESE

NOA with normal fsh (5-6) repeated several times , borderline inhibin b (94), normal tt (450-550), estradiol 14. Biopsy found some rare ocasional sperm in random tiny sample and I was diagnosed as NOA with HS / Late maturation arrest histology. So next step is mTESE as that may find pockets of mature sperm focalized tubules. No azf deletions but Y chromosome translocation is the culprit that most sperm dont fully develop.

My surgeon has prescribed me clomid 25mg per day (conservative approach). One week in and I don’t feel anything , if anything I feel I have less sex drive .

Good idea to keep taking it ? The point is to optimize testicular environment but given that my hormones are ok maybe it’s too risky ? I know my surgeon knows best but would like to know other people’s thoughts.
