Hey everyone,

I have a question regarding name formatting for a Deloitte PHT FORM.

My name on Aadhaar is in this format (surname first, which is common in some parts of India):

My name as per Aadhar:

Reddy Kumar Venkat Anil

Now the form only has 2 fields:

First & Middle Name (as per Aadhaar)

Last Name (as per Aadhaar)

I’m confused how to split this correctly.

Option 1:

First & Middle: Kumar Venkat Anil

Last Name: Reddy

Option 2:

First & Middle: Reddy Kumar Venkat

Last Name: Anil

I want to make sure it matches Aadhaar properly and doesn’t cause any issues later.

People who have faced similar situations (especially with surname-first formats), how did you fill it?
