So if you're like me, immediately right when a boss dies I quick swap to the quick step (pun intended) to run around and pick up loot. Either that or I'm using a secondary weapon/exotic for whatever reason and am not holding my Primary weapon when a level/objective completes.

This has led to my primary weapon to be severely under leveled compared to my gear in terms of augment level.

Make sure to always be holding your desired weapon you want to be leveling up whenever you see that XP completion banner pop up at the end of world activities or level completions.
