Previously met conditions for proficiency, player career, etc.

Not that the team doesn't have enough things to fix, however, there are a few steps in the proficiency ladder and player career where it should be normalized that if the player previously met the condition for completion, that completion should be awarded. Specifically, for the "upgrade ______ weapon one time". Like an idiot I immediately upgraded my first T2 AR to 50 then hit the step in the proficiency ladder where I need to upgrade it and I'm now stuck because I haven't had another one drop, and I can't craft one for whatever reason. My fault, I know, but it's still a bit of inconsistent game design as the proficiency steps that require equipping legendary mods will read the current state of equipment and award you with completion if you already have those mods equipped, instead of making you unequipped then re-equip.

Similar issue with the player career requirements to complete missions on heroic. If I've already done it, it should be immediately awarded. Not because I don't want to or I'm not going to run a particular instance again, but because there are many other steps in the progression that are retroactively awarded.

Can anyone explain what actually counts for this beyond matching the damage type on an NPC no less than 5 levels lower, as in:

is there a certain % of damage you have to deal on an special damage NPC to get "credit" for it in team mode,

does it still count for you if it was another player that got the fatal shot?

What about if it was a skill or grenade that actually got the fatal blow?

What if I don't start landing shots until after their shield is gone?

Any other conditions to be aware of?

I ask because it seems there's times when the kills aren't stacking when they should.

I searched briefly but couldn't find an answer. Is there some sort of schedule or triggering conditions for elite patrols in the open world? I've only seen one and don't know if I'm just missing them or if they're super rare or both.
