Ds-5504 question

I accidentally left my name off of my passport application and I want to have this fixed.

I called passport services twice to clarify what form I would need and both times they told me a DS-5504 even if it was my error and not theirs.

Can I use the same photo since they give you two? It was taken in May so it’s not 6 months old and within 90 days of receiving the passport.

Has anyone done this? I am afraid it will be denied for either the photo being the same or being that it was my mistake. I don’t see why the photo would be an issue but I am getting conflicting information on it.

If they reject it, do they send it all back? TYIA

Shadows in pictures

Need to send in a DS-5504 to fix my middle name being missing and got my picture done at a passport photo licensed place AAA. They said it was fine but when I further looked at it, there’s a natural looking shadow on my neck.

How strict are they with a shadow on the neck?

I went back to have them retake it because from what I understand they aren’t acceptable at all but when I got there, it closed early.

I’m so frustrated. Should I go back and ask for a new one or does it have to be rejected before they will replace it?

Forgot my middle name, would like to fix it.

Long story short, I accidentally left my middle name off of my passport and I am absolutely kicking myself over it.

I just got my passport card today and noticed my middle name wasn’t on my passport or passport card. I checked my ds-11 to confirm and noticed that I had printed out and sent the form without it that I thought I deleted and not the one with my middle name.

I called passport services and he said I needed a DS-5504 form because if I sent my birth certificate in (obviously I did) that it should match what the birth certificate says even if it wasn’t on the actual application form.

Now I’m not so sure that’s the form I need. I’m getting conflicting information. Since it was my error, some places say that I will need to re apply with a DS-11 to get my middle name on it.

Regardless, I would really like to have it fixed to match all of my documents if possible. Has anyone ever corrected this? If so, can you explain how? My passport and card were issued May 26th. How big of a deal is it if a middle name is not on your US passport?

Thanks in advance! I will absolutely be calling back tomorrow for clarification on what exactly to do as well, just wondering what those have done in this situation.
