



Need opinions: genioplasty vs jaw surgery vs doing nothing
Surgeon said my right mandibular condyle appeared larger than the left. Does that mean I have unilateral condylar hyperplasia (CH)?
I was hoping to just do genioplasty because I don’t want to commit to DJS but it seems genio won’t fix the asymmetry 😭
First consultation (OMFS):
- They took photos from all angles
- Checked my bite
- Noticed my jaw deviates to the left when I open
- Did NOT immediately recommend genioplasty
- Referred me to an oral surgeon because he said they need to understand my bite/jaw situation first
Oral surgeon findings:
- Crossbite
- Facial asymmetry with chin shifted left
- Small chin
- Jaw asymmetry/deformity noted
‼️Right mandibular condyle appeared larger than the left
- TMJ clicking + possible disc displacement + narrowed joint spaces
He told me:
- My bite itself is not severe
- If I do nothing there is no major health risk
- My concerns are mainly aesthetic
- I could technically proceed with genioplasty if I wanted, BUT he strongly suggested additional TMJ testing first
His concern was basically:
If there is a condylar/TMJ issue and I do genioplasty only, my asymmetry may remain or potentially become more noticeable because the underlying cause wouldn’t be addressed.
If I wanted to pursue the bigger route, he suggested:
Step 1: TMJ MRI (open + closed mouth) - although I have to do it in Madrid
2: Orthodontics
3: Wisdom tooth removal
4: Orthognathic surgery planning (6+ months)
I’m confused because I’m not sure what is actually causing the asymmetry.
Is it:
- the crossbite?
- TMJ?
- condyle asymmetry?
- jaw structure?
- all of the above?
Has anyone had similar findings or gone through something similar? Especially people who initially thought they only needed genioplasty but later discovered a bigger structural issue.
