People keep getting booted for low approval rate and major issue rating because reviewers can't make up their mind on what is wrong and constantly keep returning tasks with new issues instead of gathering thoughts in one setting and sending it back once.

In the past couple of days I've had a couple of tasks returned with one of them citing in the comments "This is a very minor fix, here do this ..." and the comments are labeled major issue ???
