New ortho suggested premolar extraction for revision

Context: I had djs around a year ago and wasn’t advanced properly. I consulted a new ortho and she said I could do the revision without any orthodontics but the advancement will be limited due to my front 4 lower teeth pointing out forward. If we extract my premolars we could further decompensate and make my teeth point at 90 degrees. I see that a lot of people on here advocate against extractions so I’m curious to hear thoughts.

Am I delusional for thinking I am still severly recessed 1+ year post DJS and Genio

I had djs more than a year ago with a small advancement but a big genio (check previous posts if interested in measurements). I should have never accepted that plan but didn’t know better back then and I trusted the surgeon. He says it looks good. Also saw my ortho today and she says it looks great. I already made appointments with a new surgeon and a ortho too seek second opinions and pray that insurance would cover a revision. I have lip incompetence, teeth don’t even touch when face is fully relaxed, my midline is off and when I fully open my mouth my left side of the jaw does moest of the work and opens more than the right side. Please give me your honest thoughts so I know I’m not going crazy over this.

Would I need braces again if I was to get a revision?

For context my bite is ok now but I am still recessed especially in my lower jaw, have lip incompetence etc.
