Looking for primary care doctor - Has anyone experienced this? It seems odd.

I called a doctor's office and stated that I was looking for a primary care doctor as a new patient. The woman on the phone said that they require an application to be submitted before an appointment can be made. This application is not available online, so you have to go to their office to fill it out. I asked what was included on this application, and she said it was basic questions like, "Have you had a primary care doctor before?" "What medications are you taking?" etc.

It just seems really strange, like they want to size you up for certain "desirable characteristics" if you know what I mean. Does this seem weird to anyone else?
