Need help with 5 year lookback for Medicaid (MI) and house bought during that time

Not sure if this is the right place but...

TL;DR Friends friendly grandma I had been staying with made deal for her home (under market value some depending where you look) to be paid by certain date. Grandma recently had stroke and looks like she will need assisted living/medicaid but I still owe money for house. What will happen with the home?

Main thing is, friends grandma was letting me live at her home, I am close with the family and she's like another grandma to me (and I am like one of her grandkids). The grandma wanted to sell the home to me and I agreed to 150k (under market value but most websites seem to vary if its a little or lot under value and its a older home, pre 60s) and she could stay there.

A few years ago my dad passed and left me and my siblings shares in the family company, we received a initial amount of money after everything was settled too, with the company wanting to buy the shares eventually.

I used 20k to be the initial payment for the home when we signed, with the agreement to pay the rest over the next 5 years, or as the plan was when I sold the shares (there was no real rush as she was planning on leaving it to her kids anyway). Jump ahead 3 still no sale yet (tariffs made them back off) and she had a stroke and was getting better but now had another stroke recently and its looking like she will need to go into assisted living(?think its the right term).

Probably about a month or so ago, her son told me about 5 year back check for Medicaid in Michigan, so no I am not sure where it puts me with the home. Cause if I had to, I was going to cover the rest owed by either selling the home or loan etc if it got close to the end. But after hearing about them looking back 5 years and what she asked for the house being under value some I am not sure what to do, as I don't want her penalized by medicaid for selling the home for less and unsure what would happen to the home?
