Ok, tirzepatide has been a smashing success for me. I’ve lost almost 90lbs so far (SW: 357, CW: 268, 45M, 5’11”, T2D) starting late November 2025, 7.5mg since late January. My blood lipids and A1C are normalized, and I feel GREAT. Up until a couple weeks ago, my hunger and appetite had been well controlled but recently it has come roaring back. I’m hungry all the time and the calories I had been consuming don’t sustain me in the same way, and that pervasive food noise is driving me nuts. For reference, I’m eating 2300~ calories, 200g~ protein, 150g~ carbs, >40g fiber, 90g fat per day, with time restricted 18:6 intermittent fasting. I also walk an average of 90 minutes every day, come rain or shine. Once a week or so I do stretch up to accommodate a restaurant meal or extra indulgence, so it isn’t all hammer all the time. Apple Heath says my TDEE is around 4200 calories, although this seems high to me. I should mention that I’m also taking Jardiace 25mg to treat proteinuria, which wastes glucose to urine and induces a small calorie loss, as well. This structure has served me well for the last 5-ish months. I’ve also noticed that my blood glucose, tracked by CGM, while still well controlled is more variable. I’m amongst the fortune that has had almost zero side effects to the meds so I do not fear titration. So, you veteran GLP-1ers, is this the sign it’s time for me to titrate to 10mg? Any suggestions?
