Nailed 1/3 of the way pulled out

This is the 1st step after stepping down of the 1st floor and riser

I can push this back up into position easily

I wanted to turn our Michigan basement into a better looking/feeling storage area and maybe put a basic gym down there. First thing I was going to do was fix these stairs as the first step always gives about a 1/4 an inch and sketches me out. I thought it was just a loose step. Looked at this and I'm speechless. Is this as horrific as it looks to me? I don't see a header either. It's just nailed to the top of the joist and it's 1/3 of the way pulled out. How would you even fix that? Also it's sitting in water as there's a little channel running all along the parameter emptying into a drain. Kind of like a French drain only on top for the concrete. Smh. Very overwhelmed. Any info or suggestions would be greatly appreciated.
