I'm almost to the end of the show and something keeps bugging me about house, the way he acts, reacts and takes pills - is he in pain?

I don't know if I'm being stupid but it's never been mentioned in the show, all the people sort of ignore it.

Did the show purposely frame it this way so that only the most keen observers actually notice it?

ps: Also I know house has a leg thing but I don't think he feels pain due to that since he's always using a cane on the wrong side (its also mentioned in one episode) so I don't think slight shoulder pain would be a big concern to house.
