[ACNE] 2 years of Back Acne, time for Accutane?

I’ve had Bacne for 2 years now. It started from an obvious cause (doing Physical therapy mid-work day, then sitting in my sweat, post shoulder surgery).

The first few months I didn’t really do anything about it because I couldn’t reach my back.

Over the last few years I’ve tried Salicylic, Glycolic, Adapalene, Azelaic, Benzoyl peroxide, Nizoral, head and shoulders, and Selsun Blue. I finally started Doxy 2 months ago and haven’t really seen any improvements.

I also take some supplements like Zinc and Omega 3 to help (some studies show zinc can cutdown on acne severity).

I have recently cut whey protein and am considering trying Keto and avoiding sugar and seeing if that helps.

My current routine is: Azelaic in the morning, alternate daily BP / Selsun Blue in rhe shower, and Adapalene at night.

My derm told me just to use BP + Doxy, but I feel like I have alot of scarring and read adapalene and azelaic are good for that.

This is seriously hurting my life, I never take my shirt off and avoid the beach/vacations, and don’t go on dates / need to hide my back when getting intimate.

I am thinking about going on Accutane as I really can’t deal with this much longer and it killing my confidence in alot of ways. I do read some bad things about it so am kind of nervous and I also drink a fair amount. Im not sure if I have any other options. Is there anything else I should try before accutane, or do I need to at this point?

I admittedly have some other habits that probably aren’t helping heal (Chronic Zyn usage, Weed, poor sleep, not enough water). I am trying to cut back on nicotine as I think it probably is slowing healing to some extent
