I use a $250 set of studio monitor headphones with a sound card and I have noticed that the volume of walking/running enemies is completely random. If somebody is on the floor above me, its like somebody is playing Dance Dance Revolution wearing cement shoes trying to win the annoying upstairs neighbor award, but when some snot-nosed call of duty refugee child playing on the xbox mommy bought him for getting an A- in English is zipping through buildings at the speed of fuck while hipfiring me from 0.006 micrometers away with a neon yellow M4A1, they make absolute no sound whatsoever, and yes, my in-game audio settings are normal, and I even played back a recording to confirm that an obviously sprinting enemy did indeed make absolutely no sound whatsoever. The games sound design is clearly fucky.
