What are common causes for high Macrocytosis plus low ferritin iron sat?

I have a refferal to hematology and my Macrocytosis is elevated (has been for a few years but not prior) but my ferritin is 19, iron sat 17 percent, iron (i know it has fluctuations ) 47.. my thyroid levels are normal, liver normal, non drinker, no meds that raise Macrocytosis, b12 436 (or close) folate 7.6. 36f

What other causes besides the ruled out common ones cause my combination. I was in a car accident 10 months ago and ended up with multiple injuries including 7x broken ribs. Some healed , some ended up angulated and left me with chronic inflammation. Would inflammation cause the mcv + iron deficiency

Hematology and crit or normal, sometimes low wbc 12.9 high and a few others are either mildly high or low mchc, rdw lymph absolute

Including I have undiagnosed pulmonary , and ocular issues (I see the appropriate drs fir these issues)

Posting here again because the last time everyone seemed to focused on the fact I said anemia instead of iron deficiency. Im a pt not a dr.

Curious about my level of anemia

I have an appointment coming up with a hematology/oncology dr. Im curious what you think his next step will be for testing. My abnormal blood work is high mcv 101 for 3 years , low mchc low at 32 , rdw high 13.6 ferritin 19 , iron sat 17 percent, iron 47 with b12 447, folate 7 wbc 12.6 no alcohol use, ths thyroid levels are normal, no significant blood loss. Ive been to a rheumatologist and had completely normal auto immune labs, immunologist completely normal labs except ige allergy response at 1156. Opthalmology diagnosed conditions of non viral neurotrophic keratopathy, limbal stem cell deficiency. I was told by cleveland clinic my vision loss from these issues was caused by some type of systemic inflammation. Brain mri clear. Chest ct says mosaic attenuation and I have diminished sound bilaterally in both lung bases. No infection per culture. Im 9x months post car accident where I ended up with 7 broken ribs. A few healed slowly 🐌 but a few are currently non union with inflammation (my 8 to 11 left) my face was injured in the car accident with inflammation (that I still have since 9 months ago right cheek is chronic inflammation with infraorbital nerve issue) Face ct is clear of fractures, sinuses clear of infection per ent. No dental infection per dentist. My abdominal mri is fine. my uncle recently tested positivefor MDS, pre leukemia * .... with all things considered. What will likely be my hematology drs next disease to consider or test to consider? Im trying to learn to prepare myself

36f Bilateral Severe ocular surface disease Pulmonary function issue Vision issues started in 2023 when I was taking care of my dad who passed away from stage 4 sc cancer that spread into leptomenigeal mets. Permanent vision loss at 20/200 20/400 March 2024 Pulmonary issues started in my left lung prior to car accident April 2025 Fatigue seems to be getting worse 😩

Curious if my levels mean iron deficiency iron sat 17 percent

iron sat 17 percent ferritin 19 , iron 47 36f high wbc 12.1, high platelets 347 high rdw 13.6 , high mcv 101 and folate normal per blood work 446 for b12 . I do not drink alcohol, my thyroid levels are normal. I have systemic inflammation causing ocular sufface issues, pulmonary issues. chronic inflammation post 9 months in face after injury as well. what disease will my hematology dr likley look for with my workups. The immunologist said my immune system is fine except allergy response of ige of high 1156 , Rheumatology said all of my auto immune panels are clear . no auto immune disease, mchc low 32.5 lungs to not sound like they fill with oxygen with listening on a stethoscope , diminished (no movement) sound bilateral lung bases , 36f auscultation no sound in lung bases.
