spoiler; What if a lot of the odd deaths were just intelligence officers fucking up?

I mean seriously, alot of times you hear oddball stories about x killed y over the slightest thing or other stories of what would be called an odd deaths, and what if it was an agent that freaked out because a specific thing was said, or they took something the wrong way? not only while working, but also some could be from ptsd/triggerword or actions.. People mistakenly think that the CIA is some men in black, and while they are, theyre also human. they have families, make mistakes, and feel fear. Not to mention, once you're involved in that kind of stuff, you HAVE to be hypervigilant and analytical, and if you're doing it constantly/as a job, you can run into false positives, its just mathematically inevitable.. example, you get spooked because someone gifts you a carnival cruise ship shirt as a tacky christmas gift for laughs, but theres just a freak chance that you actually were in cover and had to travel via carnival for a high profile op 2 years before that. Just crazy odd situations like that. Ive heard of certain things triggering soldiers, i cant imagine the odd things that intel officers could be triggered by.
