This is my female betta Franny, she’s has had a surface wound for over a month and is unable to heal it herself and I need a step by step

Edit! Higher range ph is 8, ammonia is 0-0.25, nitrite is 0 and nitrate is 5
The first 3 pictures are her as of recent, 4th is her after she settles and healed the notch I bought her with, fifth is after a couple days and 6th is her still driving to her new home. Also the first picture is her right side and second and third are her left side, shes eating and jumping to get food if I point to close but seems lethargic when she’s not watching me

It’s very hard to tell but around her top fin almost looks scraped on something, I have 1 rock that she sometimes likes to wedge herself between but it’s on both sides and during the pictures I took last night I noticed her scales around the wound seem to be funny looking in a way that makes them almost looks scared like she has no scales right there but still full Color?

She’s in a 15-20g heavily planted tank with several wild and blue shrimp, a few loaches, a single nitrite snail and a single assassin snail that may or may not be still alive, I typically put tannins in her tank but right now it’s basically clear, I haven’t tested the water but I can and will with the api water kit if that’s the next step, her water is slightly lower then normal but during winter is when I will put an extra heater and summer is just about to end, when this first appeared her water was at the higher temp of still a-okay for bettas and now it’s closer too the lower end but still a-okay for bettas I have aquarium salt but no hospital tank and she gets sad for the 5 minutes she sat in the water.

If I missed anything which I probably did i can clear it up when it comes up!!
And the tank has been up and running since I had my platinum long finned betta who lasted a little under 2 years so approximately 3 years stable
