Is there any way to look up the Medicare part B number (letters and numbers) for a customer? At a previous employer, we could look up entering the last 4 of their social security number.

On the self evaluation, do we give ourselves a number 1 thru 5 on each value? Or do we just state what we do to achieve each value?
