Was in a car accident last July...

Taken from my MRI;

**FINDINGS:**.

*L4-5: There is a 3 mm Concentric disc bulge. Mild to moderate bilateral facet joint arthrosis. No significant central canal or foraminal stenosis.* **L5-S1:** *There is a 3 mm Concentric disc bulge. Moderate bilateral facet joint arthrosis. Moderate bilateral foraminal stenosis and mild to moderate central canal stenosis. Bilateral pars defects at L5 without resultant subluxation.*.

**IMPRESSION:** *Multilevel degenerative changes as detailed above. There is a pars inter-articularis defect/spondylolysis of L5. Recommend MRI lumbar spine or CT lumbar spine for further evaluation. 7 mm anterolisthesis of L5 on S1. There is mild facet hypertrophy. Disc spaces: There is mild disc space narrowing.*

I've had PRP, and steroids injections in both neck and back. I'm being referred to a surgeon because of that Pars Defect.

My question;

I'm 39 this July, and if offered the surgery I have the option to decline or move forward. I'm not getting any younger and I done want this to get worse. Has anyone had success with the surgery?

LOCATION: California.

My son recently turned 18.

He would like to change his last name to mine, as the last name he has is a man I dated during my pregnancy. The man on his birth certificate has not been a part of his life since he was 2, and has no contact with him. He's also been proven not the father via DNA testing.

My question;

How do we start this process? Can we fast track it, as he would like to get his driver's license asap, and wants his last name correct. I've read we have to post in the newspaper for 4 weeks, is that still valid??

Any help is **greatly appreciated**
