I have a "Free the Captive" quest and everything was going well, I found the key, I used the key to open the cell, and there was no prisoner there to rescue?

This was in a three skull dungeon, so I checked both levels of the dungeon again to see if I could find the prisoner anywhere but couldn't find him.

Is this a known bug? I ended up just abandoning the mission but I thought perhaps I missed something that I should have done, or some fix that I could try next time?
