Which forms do I use to request Minor's Counsel for a non-verbal child with Autism? (CA)

I am posting this using a throw away account due to the sensitive nature of the subject matter.

I am the Petitioner in a restraining order case involving my two children. My 19-year-old, who has Autism, ID, and psychosis, is currently in a residential facility after assaulting my 5-year-old, who is also Autistic and non-verbal. At the time of the incident, I opted for a psychiatric hold for my daughter instead of pressing criminal charges, as my priority was getting her medical help.
There is a TRO in place, but my 19-year-old hasn't been served yet because they are in a facility. My daughter is not conserved, but she is mentally incapacitated. I have a hearing coming up, but I am not getting the help or referrals I need for her. I am frustrated and feel completely alone in this.
Because I am the mother of both, I have a major conflict of interest and want to step back from the legal advocacy. I am already filing for a Guardian ad Litem for the 19-year-old and am now looking for the correct forms to request Minor's Counsel for the 5-year-old.
My goal is to ensure my 19-year-old continues to receive the specialized care and support she needs in a safe environment, while simultaneously ensuring my 5-year-old is protected and secure. I want neutral, independent advocates for both children so I can step out of the legal battle and focus on being their mother during this crisis.
Any help with form numbers or advice on how to phrase this to the judge would be appreciated
