A simple technical writing question

When I was in college I took a technical writing course. There was a form that we were taught to use consisting of a table with cells labeled with letters of the alphabet. The purpose of the document was to establish and conform to conventions--which words to use, which font to use, etc. I want such a form for a project I'm working on, but I guess the world has moved on because I can't find much information about it.

What's the name of that form, please? I don't want anything elaborate like software I need to learn, just something I can slap together in Libreoffice will do.

When in the 20th century did men's white patent leather shoes become passé ?

I was watching a comedy from 1981 wherein a male character was being mocked for wearing white patent leather shoes with matching belt.

This got me wondering just when that look went stale. Early 70s? Late 70s? Can it be narrowed down to a single year?

Name the movie where a woman cocoons a man to steal his life force.

I just saw a trailer on IMDB for a movie ostensibly called Cocoon (1989). A woman in a ball gown (who is made up to look older) holds a young man close, and behind his back, draws a thread from her mouth in order to cocoon him. Upon emerging from the silky cocoon she's become young. Then the man's desiccated corpse falls into frame.

I have clicked everywhere on the video, but amazingly, there's no way to connect to the IMDB page for the trailer they're playing(!) I have googled "Cocoon 1989" and all I get is the Ron Howard movie or its sequel.

I assume it was released under more than one title. Anybody have the title for the Cocoon movie from 1989?
