High hot/cold/pressure sensitivity for months after filling, looking for advice

Hoping to get some opinions on how to handle the situation I'm in. Basically, last year I decided I was going to finally start getting my dental issues fixed after getting all my wisdom teeth extracted. I'm 31/M and in good general health I'd say otherwise, but my teeth aren't pristine due to some less than stellar care in my late teens-early 20's. So far I've had a root canal and 7 or so fillings done, all resin, and everything has been fine up until late last year when I got a shallow cavity filled on the side of my upper right back molar. I was met with some hot/cold/pressure sensitivity initially that was rather strong and caused lingering pain long after the source of cold/heat was removed, and it's now 5 months later and I'm still experiencing those symptoms. I also will occasionally get a painless feeling of pressure/throbbing in the adjacent gums while working out, especially if I dip my head downwards.

I went back a few months later and got a second small filling done on the top of the same molar. It has gotten slightly better over the months (doesn't randomly ache to the point of needing advil), but I still can not eat or drink on that side of my mouth. I've gone back to the dentist multiple times for reshaping of the second filling (it was sitting too high at first, it now sits perfectly) and inspecting for damage, both in person visual and via x-ray, and the dentist isn't seeing anything wrong with the fillings or any anomalies in the x-ray. She is at a loss and is advocating for a night guard, and if that fails, a root canal.

At this point I'm dubious of the night guard being of any help here as there isn't any resting pressure on that tooth even if I clench my teeth and I don't wake up with jaw pain, but I am willing to give it a shot if others think that'll have an effect. As much as I don't want to I'm about ready to go for the nuclear option and get the RC done because the discomfort and pain is really bringing me down at this point. Is there anything else you think I should try before going that rout? Thanks in advance.
