Prescriber reluctant to prescribe booster?

I’ve been titrating Elvanse and generally it’s been good - the main thing is how quick I seem to metabolise it so the duration isn’t great.

I take it at 8am most days, my work day starts at 9 and ends around 330. I’m a secondary school teacher so even though I’m in work for 715 and leave around 4, I think the most effective time to take it is so it’s active when I’m teaching. I take it with protein - usually a shake as I can’t stomach food and then at lunch I do eat a reasonable lunch - usually leftovers from dinner the night before and it’s always balanced with a mix of carbs, proteins and fats. I try to have a dense lunch but sometimes I don’t eat that much of it but I try! I also have occasions where with events, parents evenings, sports fixtures etc I’m working actively at school until much later - 7-730pm.

I don’t have a noticeable crash as such, but I do find it wears off around 2pm, this has been noticeable on all doses. I’ve mentioned this consistently at my 2 titration appointments. I started on 30, then 2 weeks of 40/2 of 50 and now I’m just coming to an end of 2 at 60 and 2 at 70.

I think 70 is a little much for me even though the duration is much more effective but I find eating tough and I can feel quite nauseous on it which makes me think it’s too high. 50/60 seems like my sweet spot.

At my last appointment, I asked about a booster as my friend who is also with Harrow Health has a 50mg Elvanse and then a 10mg Dex booster. My prescriber was reluctant to do this. On one hand, the 70 is great and it lasts much better but there’s the side effects of the nausea etc.
On the other, 50/60 does what I need it to with no side effects but it just doesn’t last as long as I need it to.

Is it worth mentioning the booster again at my final titration appt on Saturday and is there any way I can really advocate for the booster? Especially as I know my friend does have one with the same provider.
