Hellooo! I just wanted to clarify something about the Form 138 requirement. May 14 pa po release ng complete grade 12 report card ng school ko and im planning po na sa second batch (may 18) nalang po mag enroll. The report card I’ll be getting from my school on May 14, is that already the exact form 138 that ust requires? (the original with signature and dry seal), or is there a specific version or format that I still need to request from my school?
