Managing intense itchiness and the resulting damage?

Often times, my patches become so intensely itchy that it's almost painful to not itch it. The torture of such a pervasive itchy sensation is worse than any rash or bug bite I've ever gotten. Rubbing the patch ultimately results in raw patches like these that weep, are extremely tender, and, of course, are itchy. The joints on my afflicted toes feel very achy and sore as well.

Bandaids tend to come off quickly. They don't stick whatsoever if I apply ointment first.

I tried those finger cots but they cut off the circulation and make my toes sore. I tried the silicone toe covers but they're so thick that they they squeeze my toes in my shoes and make my toes and feet sore.

I hope I don't have to stress how ineffective "don't itch" is for advice.

How can I stop it from itching to begin with? How do I manage these raw weeping patches after?

I have it on my fingers and palms as well but aside from my palms splitting open, my ridiculously thick, dry, flaking, red skin all over my hands and fingers, and occasional itchiness, my hands are doing better.

Right now my toes are the main issue. Though I'm sure that once they start to heal, my hands will flare up again.
