MAT student intimidated by lesson planning: how do teachers actually know what curriculum to use?

I'm working on my MAT for elementary studies, and haven't started student teaching yet. But I'm intimidated by lesson planning. Well, I mean, lesson planning itself is fine. I enjoy it, even if it is a bit time-consuming... but how do you know what to lesson-plan around? With so many subjects and so many grade levels, what are we actually using to build the lesson plans? I understand we can take whatever our state standards are and build lessons around those, but it still seems very open-ended. Do the schools provide a curriculum that we use? Are we really expected to make a full year's worth of content out of thin air, based only on standards? How does it work?
