What do I do from here?

I honestly have no idea what I should do at this point to get into an Ivy or some top school. For context, I am a sophomore at a competitive high school, with at least 40 seniors I know getting into T20s. I want to do premed, and honestly spread myself out very thin. I am in debate, econ, a few honor societies, science olympiad, HOSA, and breakdance. I have won a few awards for my competitions, and at some local biology and chemistry competitions, but nothing on a national level. I am only an officer for econ, and I am going to be president next year. I don't know what to really do at this point. I have reached out to a bunch of professors for lab internships, but none of them agreed. I know I want to try EMT training, but I am only 15, and turn 16 late. This also limits be from a lot of shadowing and hospital volunteering. I definitely want to do either USABO or USNCO, but am not sure, because USNCO would be harder because only 2 from a school can go, while USABO seems harder because I am a little better at chemistry. In terms of my activities individually, I'm not that good at debate, haven't made ILC yet for HOSA, and breakdance is for fun. My GPA is fine, and I have taken AP Stats and APCSP last year, and I took AP micro, macro, csa, calc, and gov this year. My SAT is also decent, with a 1540. Any advice on what to do for me? Ideally some stuff to do in the summer. Thanks
