Just wanting to leave community college

I will try to keep this brief. I am about to wrap up spring semester at my local community college. My first year I got generally okay grades with one c and one d. Unfortunately due to my inability to manage my mental health last semester I failed 3/4 classes and got a d in the one I did pass.

A ton of this failing came from complete lack of motivation as I felt I wasn't working towards anything (independent studies as major) and general feelings of complete hopelessness for the future in general. This semester I am on track to do better, I am retaking the courses that I failed last year and my college will replace the old grades with the new ones. A lot of this improvement was due to taking better care of myself but mostly finally finding something to work towards. I really want to start a degree in computer engineering next year which requires transferring to a four year. I truly do believe I will be able to thrive in a new environment and having a direct degree that I can work towards.

Unfortunately although I am pretty confident I am going to get an A in three of my classes I probably am going to get a D or C in the third one despite my best efforts.

Does anyone have advice on how to better my chances of transferring this year? I have definitely shown growth by going from failing everything to passing everything. What are some ways I can make my application stand out? Is there even hope of getting in somewhere next year - even in spring semester? I am definitely willing to put in whatever work it may take.

I will be thankful for any responses, also I am meeting with a academic advisor for the school I want to get into in two days.
