Prescription label/ dispensing error England

My father was prescribed 500mg metformin tablets to be taken 2 tablets twice a day so 2000mg in total. However, the pharmacy dispensed 1000mg tablets instead. The prescription label on the box said 500mg and instructions led him to believe he was taking the correct dosage, but because the tablets supplied were double the intended strength, he was unintentionally taking 4000mg per day for around 10 days.

As a result, he became unwell and required hospital assessment. The hospital said that if I didn’t notice the error it would be catastrophic.
He has suffered symptoms including:
sickness,
upset stomach,
shaking,
severe fatigue,
worsening eyesight,
and significant effects on his mental health.

Would this be something he could try claim on if anyone would know ? Many thanks in advance

Hi I was referred to weight management in 2023 due to health and weight and I had my first appointment in march 2026. my weight specialist offered to try fast track me to having surgery as I have changed my lifestyle and lost 6 stone in the process so they said I shouldn’t need to show that I can change my lifestyle and lose weight as I already have. They had the mdt meeting at the end of march and I have a psychologist clinical appointment next week, which they told me is a one of thing because they squeezed me in and if I don’t go to this one I’d have to wait a long time. Sorry for the rant but just curious if all goes well what the waiting times could be ? I hope this was okay for me to post not hate please thanks
