Hi all, I was wondering if anyone could help me.

I have very complicated and concerning issue with my GP. They have mixed my medical data with someone that has similar name to me who also lived in the same Borough as me. They have taken me off the system when that person moved out of the Borough and when I called for an appointment they told me i moved, this happened few years ago. When I told them I never moved they told me I should look for another GP. At that time i was under medical treatment and waiting on specialist, so it was not practical for me to move GP. They even told me my information was transferred to a different GP. I had to get third party involved and after the third party contacted them on my behalf I was put back on the system but it was not done correctly. When I visited the hospital they had the other person's record so i had to correct the doctor at the hospitalmyself. I contacted my GP and the practice manager spoke with me and reassured me that my records where corrected and apologised for the mix up. After i was treated by the specialist I think i only went to the GP 2 or 3 times after that. Maybe for 2 years i did not need the GP so i never went to them.

Earlier this year i wanted to speak to my doctor about my health and when i called the GP they told me i was not registered with them since last year and i am now registered with a different GP practice. I told them i never moved GP. I didn't get any help so i had to call the NHS 111 after telling them my situation they asked for my NHS number and it turns out that the NHS number has the other person's details on it so the 111 was not able to do the assessment for me so I was sent to the A&E. The A&E also told me my record has someone else's information and i should speak to my GP.

After i got the treatment i sent formal complaint to the GP practice manager. I explained everything and how the NHS number i was given by this GP practice has someone else's details on it. Anyway the manager asked for my NHS number, I explained the situation again and told them this NHS number may not even belong to me but i gave the NHS number and my address. I asked in the email what was the reason i was removed from the GP practice and other questions. The manager sent me an email saying that i was no longer registered with them and that i am now registered with another GP practice and that i should contact my current GP. The manager also mentioned something about GDPR and how they cannot give me and information or answer any of the specific questions i asked. I explained again about the NHS number issues but the manager ignored my email i did not get any more emails.

I contacted the NHS England and PCSE they told me the practice manager can only contact them. I tried calling my local MP no luck. At this moment i am stuck. I contacted local organisation that supposed to help people with anything regaridng health care. They gave me steps to follow and told me to reregister with that GP because they have to correct my record by contacting the NHS back bank or NHS England. I went and filled in application form i explain i was returning patient, they copied my passport. After a week i was given a link to book an appointment they canceled that appointment the morning of my appointment day. I booked second appointment the following week. I went to the practice, and the receptionist tells me i am not in their record and cannot be seen by the nurse. The receptionist told me she will send an urgent email to the practice manager because no one else can fix it and i will get a call back. It's been a little over 2 weeks now and no one contacted me. At this point they do not want to deal with me or fix the problem they have created for me.

I was wondering what can i do at this point? My whole medical identify is wiped out and if anything happened to me today no one will know my medical history and that could cause serious and deadly complications.
