Dentist suggests crown on a tooth with large fillings. I've had these fillings over 15 years no issues. Nothing has came out etc. Do I really need this?

I have had older checking accounts 1+, 2+ that were closed not in use anymore. Is this something you have to list? I don't even have access to them. Also I have an LLC that I haven't touched 2+ years is this something you have to mention also?
