Light Adjustable Lens Program Fee - Is this ever covered by insurance?

I am scheduled for cataract surgery with the light adjustable lenses in July. I'm having a heck of a time figuring out all the expenses, how they bill, and what my insurance company will cover.

So far, I've figured out that I have to pay the surgery center $1000 up front before each surgery to cover the cost of the upgraded lens. They said insurance will not cover that and they won't bill it (any insurance - they didn't check mine specifically). But, they agreed to give me the billing codes ahead of time and a detailed bill after if I wanted to submit it myself. (My insurance will cover the surgery, anesthesia, etc)

The doctor's office requires $3400 per eye up front to cover the LAL program fee - adjustments, locking in, etc. They also won't submit that. I'm currently waiting to hear if they will give me the necessary info to research/submit it myself.

Has anyone every been successful getting any of the above (upgraded lenses/adjustments) covered by their insurance?
