403b beneficiary questions/issues

Myself and my 3 siblings are the beneficiaries of my deceased father's 403b. I have a few questions, as we have been told something different in every single call, and so far nothing about this process has been at ALL what we have been told over the phone.

The short version is that we were told our accounts were being created and we would each receive a "welcome packet" in the mail by the first week of May at the latest. On May 6th, I called and was told what was sent was actually a beneficiary checklist for each of us, requesting personal info, photocopies of our IDs, etc. in order to even "set up" our beneficiary accounts. He said they had already been mailed. It is now May 18, and no one has received theirs yet.

My main questions are:

Have beneficiary accounts already been set up, and this is the process of verifying our identities, or are the beneficiary accounts not even created until we return our individual documents?

Do all 4 of us have to send our documents and have these accounts set up and verified before they will move the money from our father's account, or can it be done one at a time?

I ask because one of us may take significantly longer to return their documents, and wonder if that will delay the rest of us from receiving our portion?

Another issue specifically affecting me is that my address was initially entered wrong. On the phone I was told the items the checklist asks for, and instructed to send a Fax and what to include. I sent the fax with a letter using the reference number for our case, all my father's personal information, my personal information, the required documents, and my email address with a request to send confirmation and further correspondence to my email. I have received no communication since then, and the fax was sent 2 weeks ago. Now I am unsure if this was even an acceptable way to send my docs and am unsure what else to do since I will not be receiving mail.
