Tracker screenshots and HIPAA/patient provacy

I am interested is submitting a few proposals for hourly contracts in healthcare. Forgive my ignorance, but if the hourly tracker takes screenshots while I'm working on patient records, wouldn't that be a huge HIPAA violation?

Are there any freelancer protections without these screenshots, or would it always be a gamble whether the client pays at the end of the month?
