tristar insurance group third-party claims administrator had a data breach in november 2022 that compromised personally identifiable info. they sent out notice letters around feb 1, 2024. if you got one of those notices youre eligible.

payouts include up to $500 for documented out-of-pocket expenses, up to $5,000 for actual identity theft or fraud losses, plus a flat $100 if youre in california or $40 if youre not. everyone also gets 3 years of free credit monitoring.

final approval hearing is june 23. claim deadline is july 15, 2026.
