Keeping maiden name during IVF creating issues for international surrogacy

Hi everyone! Looking for insight from any U.S. intended parents who completed surrogacy in Georgia after changing their last name through marriage (prepared embryos with maiden name and started surrogacy process with married name).

The Justice House/agency is asking for proof of my surname change from my maiden name to my married name. In the U.S., we typically just use our marriage certificate + updated IDs/passport rather than receiving a separate government-issued name change certificate like some other countries do.

For anyone who went through this successfully, what documents did you provide? Or any ideas of what I could do?

I mentioned maybe doing a “One and the Same Person” affidavit, but the agency said they’re not sure that the Justice House will accept it or not.

Trying to avoid surprises before we ship our embryos and want to be sure when it (hopefully) comes time for the birth certificate, that it’s smooth sailing. Thank you so much!
