Caremark is screwing me, what do I do?

I recently was forced to switch insurances due to a company buy-out and this switch now involves CVS Caremark as the people who control my prescriptions.

Last month my pharmacy called me saying insurance was refusing to let them fill my meds, that it was going to require a mail order or using CVS directly. I confirmed this was accurate with Caremark (after multiple calls because the first rep didn't know what she was talking about and claimed it was just one med that was not covered period, even though it had been covered by this plan prior and the app still said it was covered). I have now been trying to get my meds transferred to their mail order because I have never had a decent experience with CVS in person, for reference I'm on HRT and they are usually super judgy a**holes about it.

Caremark keeps demanding paperwork from my prescriber, claiming they are contacting my prescriber and not hearing back. My prescriber claims that they are receiving it and handling it. I ran out of meds last week, I'm on two antidepressants and one I've been without for the whole month and the second just last week. It's usually not a big deal when I go off the first, it's not an SSRI and it's really just "supplemental" but the withdrawal from the one I ran out of last week is awful. I keep playing mediator with the two entities whose job is to communicate with each other, but they literally can't communicate amongst each other without me chasing them. I can't afford to pay out of pocket for emergency fills through the old pharmacy, at least not in addition to my regular fills. What are my options at this point??

My nipples are goners, aren't they

Photos attached. At 3 weeks post op I found that my nipples smelled like rotting flesh and had a bunch of goo around the incisions, what I originally assumed was pus. I saw the doctor immediately and they put me on a harsh antibiotic for a week, it's 4 days later and they are not getting better other than the fact they don't smell as bad anymore. I've been cleaning them daily with saline spray and covering them in Neosporin (so they don't stick and have that extra antibacterial) and bandages. I'm afraid that they are dying and that the whole point of me opting for buttonhole (retaining the og blood supply and nerve connections) was wasted. My surgical team has not been sure what exactly is going on until the black scab comes off and reveals what's underneath but I'm freaking out.

EDIT: These pics are admittedly from that last appointment several days ago, I have just rebandaged and gotten a closer look (first time I could stomach investigating deeper) and it looks like this is the scab peeling off and revealing what's underneath. It is basically a small hole, but it's not quite as severe where we can fully assume it won't heal. I think it's a gamble at this point. I think my surgeon should have had me wet heal rather than dry heal :/
