Post scabies or new burrows?

Not sure whether these are new burrows forming or post scabies spots? (pictures 1 and 2 are same spot but zoomed in and out, same with pictures 3 and 4) around 2 and a half weeks post treatment, not itchy at all really unless i itch them and they’re super tiny but just not sure. Got other post scabies symptoms that i never had during my time treating (a lot itchier after, basically had no itch during or before treatment).

Burrows after treatment

I’m just over 2 weeks post 4x permethrin treatments, had post scabies (identified by a doctor) quite badly after and no new burrows anywhere but found one on the end of my elbow today. I have no idea if it’s new or old as i hadn’t checked there because it’s hard to see, it’s quite long and defined with the typical silver/ white line but not really itchy so i’m not sure. Just wondering if anyone knows how long these burrows can last before they heal and if it’s could be one that’s lingered and i haven’t seen?

Active scabies or post scabies?

At this point in my treatment, I’m mainly confused. I realised I had scabies about 7 weeks ago. I got quite lucky because I had barely any itching, but I could clearly identify a couple of burrows that were unmistakably scabies.

I treated with permethrin as standard: two applications, 7 days apart. Exactly 7 days after the first treatment, I noticed new burrows. They could have been lingering ones that hadn’t surfaced yet, but I wanted to be sure it was fully gone, so I applied the cream again that night and booked to see my GP a week later.

A week passed and everything looked better. I had no new burrows and thought I was in the clear, but I still went to the GP appointment. They said “better safe than sorry” and prescribed me 4 tubes of permethrin. I wasn’t sure what to do, because I felt clear and didn’t want another round since I have sensitive skin and I’m prone to eczema and psoriasis flare-ups (including one after the first permethrin round). To be safe, I applied the cream again that night—making it 4 separate treatments over 4 weeks.

I was going to use the remaining cream the following week, but my skin looked much better and I didn’t think I needed another treatment, so I left it.

Now I’m two weeks post that last treatment, and my symptoms have flipped. I’m itchier than at any point when I had burrows and was definitely infected, especially in the morning and at night. I’ve got no new burrows, but I do have small, skin-coloured bumps with fluid inside on my hands, and raised bumps mainly on my forearms and some on my wrists (first image). I also get specific spots that itch sporadically; when I scratch them, I get a raised welt, but that’s it—no red bump or burrow (last two images). Both these symptoms flare up and down during the day.

I’m really confused about whether this is still scabies presenting differently, or if I’m experiencing post-scabies/post-permethrin symptoms. If anyone has had a similar experience, I’d really appreciate advice—I’m going a bit insane, going from thinking I’m cured to thinking I still have it and need to see my GP again.
