Can someone explain Rule 2?

I've noticed a lot of posts being locked due to breaching Rule 2.

I would have thought "topics of intellectual interest" might have some leeway?

Can the above be defined/clarified?

You can probably tell that rules are not my forte! 😅

No tongue room post-op - posterior tongue tie release?

Pre-op I was assessed as having a posterior tongue tie. Told not enough room to release, so wait until after surgery.

Pre-op lower incisors were proclined, so ortho slightly retroclined to allow further mandibular advancement. This caused my tongue to butt up against my lower incisors, but I figured it might be temporary.

Post-op, I can only lift the front half of my tongue to the palate. The back half of my tongue is stuck to the floor. So, my tongue is still butted up against my lower incisors.

I'm 5 weeks post-op, so I realise there is still plenty of swelling (edit: and I still have a large acrylic splint in situ), but I feel like there is a whole lot of cutting required to make this right.

Did anyone else have this issue with tongue room and have it either:

  • resolve on its own?
  • have a posterior tongue tie release to resolve?

Splint question - what exactly does banding achieve in the lower jaw (custom plates)?

I'm 18 days post-op. Custom plates with large movements and 3-piece segmental Le Fort I = 7.4mm A-point and 16.6mm pogonion.

I've come to realise that my splint doesn't fit my lower jaw due to adjustments made by my ortho in the black-out period between the scans for the splint and the surgery being performed. I didn't realise the splint was fitted to the lower teeth, I thought upper only for the segmental, otherwise I would have queried the movements at the time. But it's done now, so...

ANYWAY.

My splint fits in the upper jaw, but the lower jaw doesn't fit. It clunks into place and torques a couple of teeth in extremely painful ways.

I've resorted to taking off the bands for long periods, which I know my surgeon is not happy about. I will continue the discussion with him on Tue when he is back consulting.

I've done some preliminary investigation into the purpose of the splint/bands as follows:

  1. Hold bones in place during healing:
    • I think this is, mostly, for the 3-piece segmental?
    • I think not as relevant for the lower jaw, especially having custom plates?
  2. Hold muscles in place to ensure bite comes together:
    • I think this is where it is more relevant for the lower jaw, especially if a large lower jaw advancement?

I've noticed I do have extremely tight sore muscles under my chin, so I've been massaging and applying heat and that has helped. But I've also noted that my chin actually wants to push a little further than the splint, so I guess a relapse into the retrognathic position is not presenting as a significant risk to me, in my untrained mind?

I'm in the process of trying to get the splint burred down in the places it is not fitting, in the hope I can at least get the bands back on within the next few days.

In the meantime, I'm thinking if I at least know what the consequences are, I maybe be able to mitigate them somehow.

My question is: What are the consequences of having the bands off?

For the haters: I'm not interested in any responses to "Just put the bands back on" or "Listen to your surgeon". I'm putting them on as long as I can tolerate. I frequently having massage therapists, physios and chiros tell me that I've got a very high pain threshold, so that is not the issue here. I'm also taking the panadol and ibuprofen regularly. I'm reluctant to take the opioids as this is truly a splint fit issue, not a surgical pain issue.

If it wasn't the weekend, and I know the surgeon is operating on Mondays, so I won't get a response, then I would absolutely be in ongoing discussion with him to get it sorted ASAP. I have every intention of putting the bands back on once we get the splint adjusted.
