My diabetic neuropathy Brother Case

52 year old male. Severe diabetic neuropathy with no management of A1C=12.8% since years. Heavy smoker since decades with normal weight to height ratio. No hypertension is present. Burned his toe by mistake six months ago without feeling of pain. A persistent ulcer was there and took 4 months to close (somewhat closed according to eyesight). Now developing cyanosis discoloration in bilateral feet with edema (swollen). They look hypoxic according to his description. Albumin, micro albumin, urine analysis all normal according to lab tests. No motivation to fix himself up. What's the diagnosis and prognosis?
