How do you manage your client approvals?

We run an agency managing multiple brands/accounts and one thing that always drove us crazy was that even when using our social media management platforms, we still had approvals and feedback happening through email, Slack, texts, etc.
Nothing was ever fully centralized and things would constantly get lost in the chaos:
Which version was approved?
Did the client already request edits?
Was this internal feedback or client feedback?
Who signed off on this?

We actually started building our own platform internally because of this issue and focused heavily on keeping approvals/reviews all inside the platform instead of scattered everywhere.
Curious how other agencies are handling this currently and what your biggest pain points are with approvals/workflows?
