Wheel lock lug

Took my car to a nissan dealer for a oil change and they’re saying I’m missing a wheel lock lug? How does a wheel lock lug come out! I haven’t seen the car in person yet but it kind of looks like they broke it off, also they said they misquoted me for the replacement and wanted $120
