Pain in baby tooth and upper jaw left side
hello all,
I’ll try to keep it short and sweet but I really need some help.
i have a baby tooth as my upper molar (third from the back) on my left side. it’s been giving me some trouble for the last year or two now (I have gone to dentists before with pain and they have covered exposed dentin / evaluated it), but last week, I bit down and felt some pain. over the last week it has gotten worse. it seems to be hot sensitive sometimes, and I do feel sharp pain when biting down sometimes too. I have radiating pain through the tops of my teeth (top left side) and a bit on the bottom. I went to an emergency dentist and they couldnt find any infection (indicated it could be sinuses or TMJ/tmd flare) but it has kept being painful and hurting when I chew on that side sometimes (I have avoided chewing on that side for a while now because of previous pain). I know you can’t see cracks in teeth on X Rays a lot of the time, and I wasn’t evaluated for that.
could it be a crack? A combination of things? one or the other? I’m really lost and am in pain, I just don’t know what to do. any advice would be greatly appreciated. also happy to answer more questions or give more details in the comments.

Would also like to add I do not smoke or drink, and I have celiac disease if that’s relevant. The pain is not 100% consistent either, as in I have gone a few hours here or there in little to no pain since this started
