Moving advice?

I’m planning to make a move from one state to another but I’m super nervous because I’m super young, have no current job (trying to search for remote jobs because I’m disabled). I’m mostly worried about the medical side of things, because I figure moving my stuff can be a “figure that out later” thing. I’m in a not so good home life, so I’m just trying to get a job, some money, and a ticket outta there. I’m moving in with my bsf so I don’t have to worry about housing. I just really need advice on how to go about health insurance and figuring out new doctors for my various specialty needs. Also would I need to start paying for my own phone plan if I moved states? I’m currently under my dad’s. Not a rush, I’m still likely months away from this reality, but I want to take as many steps possible to make it happen. I really want to be independent and in an environment where I feel safe and loved.
